New Patient Registration

consultation1The practice welcome new patients who live within our practice area. 

If you wish to register with this practice you need to download the Patient Registration forms (one for each Patient wishing to register), which must be completed and returned to the surgery with a photo ID:

Adult: (passport, Driving License) and a proof of address (bill or statement).

Children under 16: (Photo ID where available)

Newborn: (Not required)

This is for visual purposes only.

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire. Medical treatment is available from the date of registration.  Please contact reception for further information.

Practice Area

As with all practices we are only able to accept Patients who live within our practice area.

Our practice area covers:

  • Ratby
  • Anstey
  • Newtown Linford
  • Thurcaston
  • Groby unless you live beyond the traffic lights on the A50 at Lena Drive
  • Glenfield unless you pay your council tax to the Leicester City Council
  • Kirby Muxloe unless you  live to the right of the “T” junction at the end of Main Street i.e. Gullett Lane, Hedgerow Lane, Links Road, Desford Road, Barons Close and The Huntings or to the left of the “T” junction beyond Wentworth Green  

The following areas are outside of our practice area and as a result, we are unable to accept patients who live in any of the places below.

  • Markfield
  • Cropston
  • Heathley Park
  • Bradgate Heights
  • Kirby Fields
  • Leicester Forest East
  • Glenfield Hospital Accomodation – Glenfrith Close, Farm Drive
